For decades, orthodontists had a simple rule: if you wanted to widen the upper jaw, you had to do it in childhood. Once a patient reached the teenage years, the options narrowed to extracting teeth, accepting a narrow smile, or undergoing jaw surgery. Today, a technique called MARPE has changed that, and Strouse Orthodontics is proud to offer it to patients in Spring Hill, Lecanto, and throughout Hernando and Citrus Counties.
What Is MARPE?
MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion. It is a type of upper jaw expander that is anchored directly to the bone of the palate with small, temporary miniscrews, rather than relying only on the teeth. MARPE is a generic term and does not mean that the turns themselves will be fast or slow. In fact, today, a slow turning protocol is often the preferred method.
The upper jaw is actually made of two halves joined by a seam down the middle of the palate, called the midpalatal suture. In young children, this seam is flexible, so a traditional expander attached to the teeth can gently separate it. As we grow, that seam becomes more interlocked and resistant. A tooth-borne expander in an older teen or adult tends to tip the teeth outward instead of widening the bone underneath, which can lead to gum recession and results that don't hold.
MARPE solves this problem. Because the expander is anchored in bone, the expansion force goes where it belongs: into the skeleton of the upper jaw itself. The result is true skeletal widening in many patients who, just a few years ago, would have been told surgery was their only option.
Who Is a Candidate for MARPE?
MARPE is most often recommended for older teens and adults who have a narrow upper jaw. Common signs include:
- A crossbite, where upper back teeth sit inside the lower teeth when biting
- Significant crowding in the upper arch
- Dark spaces at the corners of the smile, sometimes called "buccal corridors"
- A high, narrow palate
- Mouth breathing or a feeling of nasal congestion that isn't explained by allergies alone
Not every patient is a candidate. Age, bone maturity, and the thickness of the palate all play a role. That is why Dr. Strouse uses 3D imaging and digital scanning during your consultation to evaluate your anatomy and determine whether MARPE is the right fit.
Why Jaw Width Matters Beyond Straight Teeth

The roof of the mouth is also the floor of the nose. When the upper jaw is widened, the nasal floor widens with it, and research has shown that many patients experience improved nasal airflow after skeletal expansion. Some patients notice they breathe more easily through their nose. While MARPE is not a stand-alone cure for sleep apnea, it can be a valuable part of a coordinated plan, and Dr. Strouse works with sleep physicians and ENT specialists when airway concerns are part of the picture.
An Alternative to Surgery for Many Adults
Before MARPE, adults needing skeletal expansion typically required surgically assisted expansion (SARPE) under general anesthesia. MARPE allows many of these patients to achieve skeletal widening in the orthodontic office, without a hospital procedure.
What to Expect During MARPE Treatment
- Consultation and 3D planning. We take digital scans and 3D imaging to assess the palate and plan precise miniscrew placement.
- Placement. The expander is placed in a single visit. After the expander is placed, an oral surgeon will place the minscrews.
- Activation. Over the following weeks-months, the expander is activated on a set schedule. Many patients notice a small gap forming between the upper front teeth. This is a normal, expected sign that the jaw is widening, and it is closed later in treatment.
- Stabilization. Once the desired width is achieved, the expander stays in place for several months while new bone fills in and matures.
- Braces or aligners. After expansion, treatment continues with braces or clear aligners to close spaces, correct crowding, and finalize the bite.
Frequently Asked Questions About MARPE
Does MARPE hurt?
Most patients report pressure in the upper jaw, nose, and cheekbones during the activation phase, which typically eases within a few minutes after each activation. Over-the-counter pain relievers are usually enough to stay comfortable.
Will it affect my speech?
Speech may feel slightly different for the first several days as the tongue adjusts to the appliance. Most patients adapt quickly.
Am I too old for MARPE?
There is no single age cutoff. MARPE has been used successfully in adults, but success depends on individual bone maturity. A consultation with 3D imaging is the best way to know.
How long does the expander stay in?
Active expansion usually takes a few weeks. The appliance then remains in place for several months to allow the new bone to stabilize before it is removed.
